We accept self-pay, commercial insurance plans, Medicare and Medicaid.

Gracepoint also offers a Discount Fee Program to all patients of the health center based on income and household size. We will care for you regardless of your ability to pay. If interested in this option, more details will be provided when you visit.

What do I bring to my first appointment?

If you are a new patient, please bring:

1. Your photo ID
2. Insurance card
3. Your co-pay
4. Bring all medications
5. Please visit the Resources link below to complete the New Patient Information Packets

If you do not currently have insurance:

Please bring proof of income for everyone in your household. We recommend that you bring your prior year tax return as proof of income. If you do not have a tax return, you may bring the following items:

  • Most recent pay stubs for one month
  • Unemployment verification or check stubs
  • Social Security payment statement
  • Social Security disability statement or check stubs
  • Retirement payments
  • Alimony payments
